After any of your visits, if you take the time to print, fill out and return the form listed below you may have your choice of $5 off or an extra 15 minutes added to your next scheduled visit. Discount or added time will be applied at next session, not session when returning the form. ONLY ONE FORM EVERY SIX MONTHS.

If during our text or call, we discuss a health condition you may currently be dealing with which I feel needs a Physicians Release; please print out the form listed below and have your physician fill it out.  It must be returned the day of your visit or the session will need to be rescheduled.

I understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, the following policies should be honored:
Please, if at all possible, give a 24 hour advance notice when canceling an appointment. This allows the opportunity for someone else to schedule an appointment. I too will do my best to inform you if I need to reschedule due to illness or emergency.
Reminders
I will do my very best to send out reminders the night before your scheduled appointment.  However, I request that you make note of your next appointment in your calendar as sometimes I work late and prefer not to send out notices too late.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, we will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration for other clients, please plan accordingly and be on time.
